First-Time Passport Check-list

To apply for a passport for the first time, come in person to the Consular Section of the U.S. Embassy in Yaounde during regular ACS hours and bring the following materials:

  • Completed Application for U.S. Passport (DS-11). Print the online application and bring to Consular Section or visit the Consular Section for the form.
  • Birth certificate (original or certified copy)
  • Consular Report of Birth Abroad (CRBA) if applicant was born a U.S. citizen outside the U.S. (original or certified copy)
  • One 2”x2” photograph, can be color but background must be white
  • Form of official identification for applicant if applicant is no longer an infant
  • Passport fee for applicant over 16 years of age $97.00 or FCFA equivalent
  • Passport fee for children under 16 years of age: $82.00 or FCFA equivalent

When the applicant is a child younger than 14 years old:

  • Both parents need to be present with the child and sign the application in front of Consular Officer
  • Both parents need to present their passport or other official photo identification containing their signature
  • If one parent not present due to extenuating circumstances: notarized original Statement of Consent form (DS-3053) completed by absent parent with explanatory statement and copy of parent’s notarized photo ID with signature (passport or drivers license etc.)
  • If parents are divorced: an original or certified copy of divorce decree
  • If one parent – or another individual – has sole legal custody, the original or certified copy of the court decree indicating this
  • If one parent is deceased: an original or certified copy of the death certificate
  • If minor child & guardian is caring for the child, (a) notarized letter(s) from both parents (or from the individual with legal custody of the child) giving permission to obtain a renewal passport.

Forms: Forms related to the passport application are available through the U.S. Embassy website, at http://travel.state.gov/passport/forms, or in the Consular Section at the Embassy.

Fees: For services requiring a fee, please note that fee payments may be made in USD or the equivalent in FCFA.  Only U.S. currency printed after 1996 will be accepted.

American Citizen Services (ACS): ACS hours are Monday-Thursday, 8:00 a.m. – 3:00 p.m. and Friday, 8:00 a.m. – 11:00 a.m.  For more information regarding, see the U.S. Embassy website at http://yaounde.usembassy.gov
